OFFICIAL RULES – NO PURCHASE NECESSARY TO ENTER OR WIN
1. HOW TO ENTER: To enter the contest, you must follow the instructions found at http://www.hybridmom.com/recipe-contest-rules , fully complete the form provided at http://www.hybridmom.com/node/add/recipe-contest-enter and submit a photograph by 11:59 p.m., central time, on June 18, 2010. NO PURCHASE IS NECESSARY TO ENTER THE CONTEST AND A PURCHASE WILL NOT INCREASE AN ENTRANT’S CHANCES OF WINNING.
Only one entry per person will be accepted. The submission is the sole responsibility of the entrant. Only entry forms actually received by the Sponsor will be included in the Contest; proof of sending does not constitute proof of actual receipt by the Sponsor. Illegible, forged, altered, incomplete or otherwise unintelligible entry forms will be disqualified. The Sponsor is not responsible for late, lost, misaddressed or not received entry forms, in each case due to any reason whatsoever. All entries become the property of the Sponsor and none will be acknowledged or returned.
2. SPONSOR: Moms for Profit, LLC, 169 Commack Road, Suite H, Commack, NY 11725.
3. WINNER SELECTIONS AND NOTIFICATION: From May 19, 2010, through 11:59 p.m., central time, on June 18, 2010, all entries will be posted on www.hybridmom.com and visitors to that web address may vote for the entry that they consider the most deserving of a makeover. One vote per person will be accepted and duplicate votes will be disqualified. Any recipient that is found to be compensating individuals for voting for them will be disqualified. Compensation is defined as cash or goods. At the end of voting, the entry that receives the most votes will be the winner. In the event of a tie, Sponsor will select the winner in its sole and absolute discretion. Decisions of the Sponsor shall be final and binding in all respects. Voting results are unofficial until verified by Sponsor. Odds of winning depend upon the number of valid entries received by the Sponsor and other factors. Winners will be notified by e- mail. Sponsor may choose, in its sole and absolute discretion, to terminate, modify or suspend the Contest at any time.
4. PRIZES: One grand prize will be awarded to the contest winner. The first place winner will receive an Todd English original cook set. The second place winner will receive a Heavenly Hostess set of apron and gloves and the third place winner will receive a Todd English cookbook. The contest winner will be notified by email and the grand prize will be delivered approximately 30 days from date of winner verification by Sponsor. Sponsor reserves the right to substitute a prize (or portions thereof) of equal or greater value if the grand prize (or portions thereof) cannot be awarded as described for any reason. Sponsor will determine all elements of prizing in its sole discretion and the grand prize must be accepted as awarded. The right to the grand prize may not be assigned, transferred, or changed, except at the sole discretion of Sponsor. Prize is awarded “as is” with no warranty or guarantee, express or implied. Sponsor disclaims all and any liability for the actual provision, quality or nature of any third party product or service accepted by the contest winner. Entrants agree to be bound by Official Rules and agree that if winner fails to provide proof of identity, refuses to provide required affidavit or other documentation required by Sponsor, is found to have violated the Official Rules or otherwise does not meet eligibility criteria, the grand prize will be forfeited and awarded to the entry with the next highest number of votes. Entrants understand that Sponsor is not liable for injuries, losses or damages of any kind arising from participation in this promotion and acceptance, possession and use of grand prize. Sponsor is not responsible for any typographical or other error in the printing of the offer, administration of the contest or in the announcement of the prize.
5. ELIGIBILITY: The Contest is open to legal residents of the United States who are at least 18 years of age at the time of entry. Employees and all household (related or not) and immediate family members of Moms for Profit, LLC, its affiliates, agents, advertising and promotional agencies or any entity involved in the development, product implementation or administration of the Contest are not eligible to participate or win.
6. CONDUCT: By entering the Contest, entrants agree to comply with and be bound by the Official Rules and winning is contingent upon fulfilling all such requirements. The Official Rules will be posted at http://www.hybridmom.com/recipe-contest-rules throughout the Contest period or may be obtained by sending a self-addressed, stamped envelope to the Sponsor’s address. Failure to comply with these Official Rules will result in disqualification from the Contest.
7. WINNERS: The contest winner will be announced on the contest web address. For a written statement of the winner, send a self-addressed, stamped envelope to Moms for Profit, LLC c/o HYBRID MOM MOTHERS DAY MAKEOVER, 169 Commack Road Suite H, Commack, NY 11725.
8. PRIVACY: By entering and providing the required entry information, you acknowledge that Sponsor may also send you information, samples or special offers it believes may be of interest to you about its publications or other complementary goods or services offered by Sponsor. Information collected from online entrants is subject to Sponsor’s privacy policy . IF YOU DO NOT WISH TO SHARE YOUR INFORMATION, PLEASE DO NOT ENTER THIS PROMOTION.
9. GENERAL RULES: This Contest is void where prohibited or otherwise restricted by law. All federal, state and local laws and regulations apply. Taxes and fees on prizes, if any, are winners’ sole responsibility. This Contest is governed by the laws of the State of New York Winners must accept the prize within 20 days of notification and are required to provide, in the discretion of the Sponsor, proof of identity, an affidavit of eligibility, liability release, and, where legal, a publicity release. Noncompliance will result in disqualification and another winner will be randomly selected from eligible entries received by the Sponsor. Sponsor assumes no responsibility for entries it is unable to process due to network, hardware or other technical failures; or any other reason, or incomplete, damaged, misdirected, or lost entries. Sponsor reserves the right, in its sole discretion to disqualify any person tampering with the entry process, the operation of the web site or otherwise in violation of these Official Rules. BY ENTERING THE CONTEST, EACH ENTRANT ACCEPTS THE CONDITIONS AND TERMS PROVIDED IN THESE CONTEST RULES. BY ACCEPTING A PRIZE, A WINNER AGREES TO HOLD THE SPONSOR, ITS AFFILIATES, AGENTS, DIRECTORS, EMPLOYEES, REPRESENTATIVES, ADVERTISING AND PROMOTIONAL AGENCIES OR ANY ENTITY INVOLVED IN THE DEVELOPMENT, PRODUCT IMPLEMENTATION OR ADMINISTRATION OF THE CONTEST, THEIR SUCCESSORS AND ASSIGNS, HARMLESS FROM AND AGAINST ANY AND ALL CLAIMS AND LIABILITY, LOSS OR DAMAGES ARISING FROM OR IN CONNECTION WITH THE AWARDING, RECEIPT AND/OR USE OR MISUSES OF THE PRIZE OR PARTICIPATION IN AN PRIZE-RELATED ACTIVITY. If, for any reason, the Contest is not capable of running as planned, including any events or other causes beyond the control of the Sponsor, which corrupt or affect the administration, security, fairness, integrity or proper conduct of the Contest, the Sponsor reserves the right in its sole discretion to cancel, terminate, modify or suspend the Contest and select winners from among all eligible entries received prior to the cancellation.
